Release Soccer Alliance, LLC. Camps

I hereby permit my child to participate in the Soccer Alliance, LLC. camps (the “Camp”). I represent that to the best of my knowledge, my child is in good health and is able to participate in Camp activities. I agree that the Camp may photograph and/or videotape my child during the Camp and that the Camp shall have the right to use these images for future commercial and noncommercial purposes without compensation to my child or me. I hereby release Soccer Alliance, LLC. and all person employed or working thereunder from liability claims and demands for any injuries or illness incurred at Camp. I hereby assume complete financial responsibility for any personal injury or property damage created as a result of an intentional or negligent act of my child or while he or she is attending the Camp.
